2. This form should be used only to apply for new residential utility service for electricity, water, and garbage/recycling collection. You must have a valid email address to use this form. To establish residential utility service, do the following:

   • Complete this form.
   • Provide a valid Texas driver's license or Texas state ID number. If you have an out-of-state license or ID, you must either email a copy to customer service, fax a copy to 972-205-2100, or bring it to Customer Service at:
     217 N Fifth Street
     Garland, TX 75040
   • Within 30 days of your service being established, you must provide proof of a valid Texas Drivers License or Texas State ID, which you can do by calling 972-205-2671, or by faxing or bringing a copy to Customer Service.
   • Satisfy the deposit requirement with a paid deposit, credit check or an acceptable letter of credit.
   • A $30 Account Establishment Fee will appear on your first bill.

   
   

   We will review your request and respond to you as soon as possible during our normal business hours of 8 a.m. to 5 p.m. Monday through Friday.
